Medicines optimisation (…) is a patient-focused approach to getting the best from investment in and use of medicines that requires a holistic approach, an enhanced level of patient centred professionalism, and partnership between clinical professionals and a patient. Medicines optimisation is about ensuring that the right patients get the right choice of medicine, at the right time. (…) the medicines optimisation approach will require multidisciplinary team working to an extent that has not been seen previously. Healthcare professionals will need to work together to individualise care, monitor outcomes more carefully, review medicines more frequently and support patients when needed.
